Community Choice Guide (CCG)

Contact your Case Manager for eligibility! 

CCG is a Washington State program created with the intention of supporting individuals with maintaining and/or gaining independence through a variety of services. This program aims to support those going through a community transition, provide stabilization for those already in a community living situation, and/or assist with moving into less restrictive community settings. 


Some of the services offered through this contract include but are not limited to;

Identify and coordinate resources needed for successful integration into the client's chosen community-based setting.

Assist with locating accessible and affordable housing options, including assistance with filling out paperwork.

Help connect the client with community activities.

Assist with setting up necessary services when transitioning into a new living situation.

Purchasing necessary items for the client's home.

Purchasing regular grocery and self care items. 

LifeServices

Life Guidance Services 

Private Pay | Health Savings Accounts (HSA) and VEBA Accepted | No Insurance  |  $75 per hour

Life coaching services designed to help individuals better understand themselves, clarify personal goals, and develop actionable strategies for growth and fulfillment.

Community Respite

DDA Waiver Respite Hours Funded Service | Private Pay Available | Private Pay Rate: $27.50 per hour

Group-setting respite activities held in the community, with a monthly activity schedule posted in advance. Open to the public with a caregiver present; participants are responsible for their own transportation. Private pay is available for those not utilizing DDA waiver funding.

DDA Waiver Funded Services

Services and programs are tailored to each client's specific needs and circumstances.

Life Skills

(formerly Specialized Habilitation)

This program supports clients in developing individualized goals across areas including communication, self-regulation, self-advocacy, social and relational safety, and adapting to everyday life changes and challenges.

Community Engagement

This program is focused on helping clients explore and connect with their community. Supports clients in developing goals to participate more fully in group settings, clubs, volunteer opportunities, and other community activities, with the goal of enriching daily life and fostering meaningful engagement.

Staff and Family Consultation

This program provides education and consultation to families and care staff. Services include guidance on current plans of care, communication strategies and techniques, building rapport, disability education, and collaboration with medical professionals regarding behaviors and observations.

Supported Parenting

This program provides parenting education and support to individuals with a developmental disability. Services address the daily responsibilities of raising a child, as well as strategies for building and strengthening relationships with adult children.

Crisis Prevention and Intervention Services (CPIS)

A specialty service providing support for clients currently residing in a hospital, treatment facility, or other secure setting. Services focus on stabilization and aiding in a successful transition back to a community-based setting. Must be specifically requested and approved by DDA Headquarters. Note that funding for this service is extremely limited.

 

Stabilization: Staff and Family Consultation & Life Skills

A specialized service intended to support clients when their current placement is at risk of disruption. 

Private Pay Tutoring Services

Private Pay Rate: $65 per hour

Academic tutoring services supporting clients preparing for the driver's education written exam, high school graduation requirements, or equivalency degree programs (GED).

Foundational Community Supports (FCS)

Medicaid Funded Service | Open to Individuals Age 16 and Older

This program is available to anyone experiencing or living with a barrier to stability, including substance use disorder, ADHD, autism, depression, PTSD, a high Adverse Childhood Experiences (ACEs) score, or a physical or developmental disability. Services are provided in two areas:

  • Employment: Assistance navigating the educational system, mental health system, and job market. Supports clients in addressing barriers and building the skills needed to find and maintain gainful employment or pursue further education.
  • Housing: Assistance locating and maintaining safe, stable, and appropriate housing. Includes advocacy with landlords and support in identifying funding sources tailored to each client's specific needs.

How are services funded?

Services including Life Skills, Community Engagement, Staff and Family Consultation and Supported Parenting are DDA funded. For Foundational Community Support, it is funded through Medicaid. 

We do not have take insurance as payment. 

What is DDA?

DDA stands for "Developmental Disabilities Adminstrations," which is part of Washington State Department of Social and Health Services (DSHS). 

The DDA helps children and adults with intellectual or developmental disabilities receive services and support so they can live as independently and safely as possible in their communities. 

Do you have female providers and staff?

Yes, we have a wide range of providers including female and Spanish-speaking providers.
